Caraway Music Systems DJ Contract

     This is a contract between Caraway Music Systems, here after known as the part of the first part, and the party of the second part ( as identified above ), to provide DJ Services according to the above listed parameters.
     The fee, less deposit is to be paid in full by the part of the second part no later than the set up time stated above. A deposit of 50% of the contracted total fee above must be returned with a signed copy of this contract, in the form of a check or money order, unless waived by the party of the first part in advance, and acknowledged on this form. Payment by Visa or Mastercard may be arranged in advance.
     By signing this contract the party of the second part will be held liable for any damage to the equipment, gear, or personnel of the party of the first part, not caused by the party of the first part. The party of the second part recognizes this document as a legal and binding contract in the State of Missouri. The party of the first part is not liable for any accidents, or injury caused by the music, lighting, sound equipment, or it’s personnel. NOTE: Caraway Music Systems may use strobe lights, fog machines, lasers, and other special effects lighting. These lights may have adverse effects on people with epilepsy.
     The party of the second part must provide at least one 8 - foot table and two chairs, as well as access to at least one 15 amp A.C. circut within fifty feet of the set up site. Due to the caliber of the professional equipment used by Caraway Music Systems, we must have at least one full hour prior to the start time, to set up and test equipment. We also require at least one full hour for tear down and load out. Caraway Music Systems reserves the right to hang a banner up within 20 feet of our equipment table. Playing To The Walls: Caraway Music Systems reserves the right to end the contracted playing time early in the event of ‘Playing To The Walls’. 'Playing To The Walls' may result if number of people in attendance falls to less than 15 people, for a period no less than 30 minutes. The first hour of any event is exempt from this rule. This contract may be deemed void only by an act of God, the terms and conditions of said contract, or at the mutual agreement of both parties.

PLEASE MAKE ALL CHECKS PAYABLE TO: CARAWAY MUSIC SYSTEMS, c/o TODD E. CARAWAY. SEND A SIGNED COPY OF CONTRACT, DEPOSIT, AND ANY ATTACHED FORMS BACK TO CMS AT THE ADDRESS ABOVE.
**CONTRACT MUST BE RETURNED WITHIN 10 DAYS OF DATE ON CONTRACT OR IT MAY BE DEEMED VOID BY CARAWAY MUSIC SYSTEMS.
